Collected Personal Data

- we will collect contact information including name, address, email and telephone number for the purpose of arranging consultations/assessments and for billing purposes.

- we may also request contact information for a parent/guardian in the case of consultation/assessment of a minor

- we will request sight of previous medical reports if you are happy to provide these

- we will issue a questionnaire to request information which is relevant to the request for a consultion or assessment.

- please let us know if any information is inaccurate or changes so that we may update our records

 - with the exception of contact information in relation to billing, all data is routinely deleted after seven years in accordance with guidelines set by the British Psychological Society

Purpose of collecting data

- all records are confidential and held securely, and are held solely for the purpose of providing tailored and specific advice during consultations and/or assessments

-in the case of assessment of or consultation with a minor whose parents are seperated/divorced, a copy of the assessment will be provided to both parents on request

-information is never shared with any third party unless required to do so either by regulatory authorities, or in order to safeguard your or your child's safety.
